If you are a new patient to St. Clair Medical Group Cardiac Surgery, we ask that you provide the office with your previous health records prior to your appointment date. To transfer your records, please complete the Medical Record Transfer Form and send it to your current primary care provider. Additionally, we ask that you please print and complete all of the "New Patient Forms" on the Forms Page. Once the forms are complete, either fax, mail, or bring the completed forms to your appointment. If you are unable to do this prior to your appointment, please arrive at least 15 minutes early to complete the forms and allow our staff to enter them into your electronic health record.

St. Clair Medical Group Cardiac Surgery is open 8:30 am to 5:00 pm. For non-emergency care when our office is closed, we recommend St. Clair Urgent Care, which is located at St. Clair Hospital Outpatient Center - Village Square, 2000 Oxford Drive, Bethel Park, PA 15102 and is open 7 days a week 9:00 am-9:00 pm. If the event of an emergency, please call 9-1-1.

If you require a referral to see a specialist or to have testing our office will help you obtain the necessary insurance authorization. When possible, please notify our office at least 10 days in advance to allow time to process the paperwork. We ask that you please do not schedule your appointment before our office contacts you with the authorization. If the authorization is not obtained before the appointment, it may mean that you could be financially responsible.
In most cases blood specimens can be drawn right in the office. If you require diagnostic testing please visit St. Clair 's Dunlap Family, Village Square and Peters Township Outpatient Centers. Hospitalists are physicians whose primary professional focus is the general medical care of hospitalized patients.
