How do I prepare for my first appointment?

If you are a new patient to St. Clair Medical Group Colorectal Surgery, we ask that you provide the office with your previous health records prior to your appointment date. To transfer your records, please complete the Medical Record Transfer Form and send it to your current provider. Additionally, we ask that you please print and complete all of the "New Patient Forms" located on the Forms Page. Once the forms are complete, either fax, mail, or bring the completed forms to your appointment. If you are unable to do this prior to your appointment, please arrive at least 30 minutes early to complete the forms and allow our staff to enter them into your electronic health record. Patients having colonoscopies are required to submit their new patient forms at least 48 hours prior to the time of a scheduled procedure or it will be rescheduled.

What should I do if I need to cancel and / or reschedule my appointment?

If you cannot make your scheduled appointment, we request that you provide the practice with a 24 hour notice, so that we may offer that appointment time to another patient. You can cancel / reschedule your appointment by calling our office at (412) 572-6192.

What is the office's late / cancellation policy 

Out of consideration to our other patients, if you arrive more than 15 minutes late for your appointment, there may be a delay in your visit, or you will be asked to reschedule your appointment. If you cannot make your appointment, we request 24 hours notice to offer that appointment time to another patient. What should I do if I need care after hours?

St. Clair Medical Group Colorectal Surgery is open from 8:30 AM - 4:30 PM Monday through Friday. If you are a current patient with an after hours medical issue, please call the office at (412) 572-6192 to have the physician on call paged. If you are experiencing a true medical emergency, please dial 9-1-1 or proceed to the nearest emergency room.

How do I get a prescription medication refilled?

Please contact your pharmacy with all prescription medication refill requests.

What if I need a referral to another provider or service?

If you require a referral to see a specialist or to have testing our office will help you obtain the necessary insurance authorization. When possible, please notify our office at least 10 days in advance to allow time to process the paperwork. We ask that you please do not schedule your appointment before our office contacts you with the authorization. If the authorization is not obtained before the appointment, it may mean that you could be financially responsible.

What if I need to be hospitalized?

St. Clair Medical Group (SCMG) Colorectal Surgery physicians trust the St. Clair Hospitalists while their patients are hospitalized at St. Clair Hospital. Hospitalists are physicians whose primary professional focus is the general medical care of hospitalized patients. St. Clair Hospitalists work with the SCMG Colorectal Surgery physicians to ensure the coordination of your care.
