How do I prepare for my first appointment?

If you are a new patient to St. Clair Medical Group General Surgery, we ask that you provide the office with your previous health records prior to your appointment date. To transfer your records, please complete the Medical Record Transfer Form and send it to your current primary care provider. Additionally, we ask that you please print and complete all of the "New Patient Forms" located on the Forms Page. Once the forms are complete, either fax, mail, or bring the completed forms to your appointment. If for some reason you are unable to do this prior to your appointment, please arrive at least 15 minutes early to complete the forms and allow our staff to enter them into your electronic health record.

What should I do if I need to cancel and / or reschedule my appointment?

If you cannot make your scheduled appointment, we request that you provide the practice with a 24 hour notice, so that we may offer that appointment time to another patient. You can cancel / reschedule your appointment by calling our office at (412) 942-7880.

What is the office's late / cancellation policy? 

Out of consideration to our other patients, if you arrive more than 15 minutes late for your appointment, there may be a delay in your visit, or you may be asked to reschedule your appointment. If you cannot make your appointment, we request 24 hours hours notice to offer that appointment time to another patient. What should I do if I need care after hours?

St. Clair Medical Group General Surgery is open 8:30 am to 4:30 pm Monday through Friday. If this is a condition that is of an urgent need and can not wait till the office re-opens, please call our office at (412) 942-7880 and our call service will reach a physician who is on call.

How do I get a prescription medication refilled?

Please contact your pharmacy with all prescription medication refill requests.

What if I need a referral to another provider or service?

If you require a referral to see a specialist or to have testing our office will help you obtain the necessary insurance authorization. When possible, please notify our office at least 10 days in advance to allow time to process the paperwork. We ask that you please do not schedule your appointment before our office contacts you with the authorization. If the authorization is not obtained before the appointment, it may mean that you could be financially responsible.
