How do I prepare for my first appointment?

If you are a new patient referred by another physician, we ask that you provide the office with your health records and testing from that physician. To transfer your records, please complete the Medical Record Transfer Form. Additionally, we ask that you please print and complete all of the "New Patient Forms" on the Forms Page. Once the forms are complete, either fax, mail, or bring the completed forms to your appointment. If you are unable to do this prior to your appointment, please arrive at least fifteen (15) minutes early to complete the forms and allow our staff to enter them into your electronic health record.

What should I do if I need to cancel and / or reschedule my appointment?

If you cannot make your scheduled appointment, we request that you provide the practice with a twenty-four (24) notice, so that we may offer that appointment time to another patient. You can cancel / reschedule your appointment by calling our office at (412) 942-5710.

What is the office's late / cancellation policy 

Out of consideration to our other patients, if you arrive more than fifteen (15) minutes late for your appointment, there may be a delay in your visit, or you may be asked to reschedule your appointment. If you require a referral to see a specialist or to have testing our office will help you obtain the necessary insurance authorization. We ask that you please do not schedule your appointment before our office contacts you with the authorization. If the authorization is not obtained before the appointment, it may mean that you will be financially responsible.
